Occupational Therapist - Home Health, Senior Living

📋 Description

• Perform outpatient therapy evaluations for adults in senior living or group homes.
• Use evidence-based approaches to maximize functional independence.
• Work with home health therapy to transition care to outpatient when appropriate.
• Educate clients, families, and caregivers on care plans.
• Conduct evaluations, treatments and discharge planning via the outpatient EMR.

🎯 Requirements

• NJ Occupational Therapist license.
• Prior home care experience preferred but not required.
• At least one year of OT experience.
• Graduation from an OT program accredited by AMA/AOTA.
• Ability to work independently and manage time effectively.
• Solid computer skills; EMR experience preferred.
