Occupational Therapist - Home Health, Senior Living

📋 Description

• Perform outpatient therapy evaluations for seniors in designated living communities.
• Deliver OT to maximize functional independence using evidence-based care.
• Coordinate with home health to transition to outpatient services.
• Educate clients, families and caregivers on care plans.
• Conduct evaluations, treatments and discharge planning in EMR.
• Full-time or per diem OT serving Somerset, Middlesex & Monmouth counties.

🎯 Requirements

• NJ Occupational Therapist license.
• At least 1 year OT experience (home care preferred).
• Graduate from an OT program accredited by AMA/AOTA.
• Ability to work independently; strong time management.
• Strong interpersonal, organizational and problem-solving skills.
• Solid computer skills; EMR experience preferred.
