Occupational Therapist- Home Health, Senior Living

📋 Description

• Perform outpatient evaluations to plan care for adults in designated senior living communities or group homes.
• Operate at top of licensure using current evidence-based approaches to maximize functional independence.
• Coordinate with home health therapy partners to transition care to outpatient services to meet client goals.
• Educate clients, families, and caregivers on care plans to maximize independence.
• Use outpatient EMR to document evaluations, treatments, and discharge planning.

🎯 Requirements

• A current NJ Occupational Therapist license.
• One year of OT experience.
• Accredited OT program completion (AMA/AOTA).
• Ability to work independently and manage time.
• Strong interpersonal and organizational skills, and problem-solving.
• EMR experience preferred.
