Occupational Therapist- Home Health, Senior Living

📋 Description

• Perform outpatient therapy evaluations to plan care for patients in senior living or group homes.
• Operate at top of licensure using current evidence-based care.
• Collaborate with home health partners to transition care to outpatient services.
• Educate clients, families and community caregivers on care plans.
• Perform evaluations, treatments and discharge planning using the outpatient EMR.

🎯 Requirements

• NJ Occupational Therapist license is current.
• Prior home care experience preferred but not required.
• At least one year of OT experience.
• Graduation from an OT program accredited by AMA and AOTA (transcript/diploma).
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
