Make home visits to clients in designated geographic territories.
Evaluate functional status incl. muscle function, endurance, coordination, ADL/IADL.
Assess cognitive, psychological and motor aspects; develop individualized plans.
Evaluate home environment for hazards to independence.
Identify equipment needs and environmental adaptations.
Continuously assess and revise the OT care plan; collaborate with nursing.

🎯 Requirements

A current Pennsylvania Occupational Therapist license.
Prior home care experience preferred, but not required.
Minimum of one year of recent OT experience.
Graduation from an OT program accredited by AMA/AOTA.
Ability to work independently and manage time effectively.
Solid computer skills; EMR experience preferred.
