Occupational Therapist - Home Health (Per Diem)

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform evaluations of functional status, including muscle function, endurance, ADL and IADL.
• Assess cognitive, psychological and motor performance; develop intervention plans.
• Evaluate home environments for hazards or barriers to independence.
• Identify equipment needs and environmental adaptations.
• Document observations and interventions using a touch pad tablet.

🎯 Requirements

• A current VA Occupational Therapist license.
• Prior home care experience preferred, but not required.
• At least one year of recent OT experience.
• Graduation from AMA and AOTA accredited OT program.
• Ability to work independently and manage time effectively.
• Solid computer skills; EMR experience preferred.
