Make home visits to clients in designated geographic territories.
Evaluate functional status: muscle function, endurance, visual coordination, and ADL/IADL.
Assess cognitive, psychological and motor performance; develop intervention plans.
Evaluate home environment for hazards or barriers to independent living.
Identify equipment needs and environmental adaptations for enhanced outcomes.
Update OT care plan and coordinate with nursing in the multidisciplinary team.

🎯 Requirements

A current DE Occupational Therapist license.
Graduation from an OT program accredited by AMA/AOTA.
Ability to work independently and manage time effectively.
Strong interpersonal, organizational, and problem-solving skills.
Solid computer skills; EMR experience preferred.
