Make home visits to clients in designated geographic territories.
Evaluate functional status: muscle function, endurance, coordination, and ADL/IADL.
Assess cognitive/psychological/motor aspects; develop individualized intervention plans.
Evaluate home environment for hazards or barriers to independence.
Identify equipment needs and environmental adaptations.
Educate patients and families; document care and evaluations using EMR/tablet.

🎯 Requirements

Current DE Occupational Therapist license.
Graduation from an OT program accredited AMA/AOTA.
Ability to work independently and manage time effectively.
Strong interpersonal, organizational and problem solving skills.
Solid computer skills; EMR experience preferred.
