📋 Description

  • Make home visits to clients in designated territories.
  • Evaluate patients’ functional status (muscle, endurance, coordination, ADL/IADL).
  • Assess cognitive, psychological and motor performance; plan interventions.
  • Evaluate home environment for hazards to independence.
  • Identify equipment needs and environmental adaptations.
  • Revise care plans; collaborate with nursing on the multidisciplinary team.

🎯 Requirements

  • A current VA Occupational Therapist license.
  • Home care experience preferred, but not required.
  • A minimum of one year of recent work experience as an Occupational Therapist.
  • Graduation from an AMA/AOTA-accredited OT program (transcript/diploma).
  • Ability to work independently and manage time effectively.
  • Solid computer skills; EMR experience preferred.
