Occupational Therapist- Home Health

📋 Description

• Make home visits to clients in designated territories.
• Evaluate functional status and ADL/IADL performance.
• Assess cognitive, psychological, and motor skills; plan care.
• Evaluate home environment for hazards or barriers to independence.
• Identify equipment needs and environmental adaptations.
• Update and revise OT care plans; collaborate with nursing.
• Educate patients and families on rehabilitation and safety.
• Document observations and interventions using EMR/tablet.

🎯 Requirements

• Current VA Occupational Therapist license.
• Minimum of one year of OT experience.
• Graduation from an OT program accredited by AMA and AOTA.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
• Prior home care experience preferred, but not required.
