Occupational Therapist- Home Health

📋 Description

• Make home visits to clients in designated territories.
• Evaluate functional status including ADL/IADL.
• Assess cognitive, psychological and motor aspects; create plans.
• Evaluate home environment for hazards to independence.
• Identify equipment needs and environmental adaptations.
• Update OT care plan with nursing in the MDT.
• Educate patients and families in rehabilitative care.
• Document care using EMR on a tablet.

🎯 Requirements

• Current VA Occupational Therapist license.
• Prior home care experience preferred but not required.
• Minimum one year recent OT experience.
• Graduate of AMA/AOTA-accredited OT program.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, problem-solving skills.
• Solid computer skills; EMR experience preferred.
