📋 Description

  • Make home visits to clients in designated territories.
  • Evaluate patients’ functional status and ADL/IADL performance.
  • Assess cognitive, psychological, and motor aspects; develop intervention plans.
  • Evaluate home environment for hazards and barriers to independence.
  • Identify equipment needs and environmental adaptations.
  • Document observations and interventions in EMR; educate patients and families.

🎯 Requirements

  • A current VA Occupational Therapist license.
  • Prior home care experience preferred, but not required.
  • A minimum of one year of recent work experience as an Occupational Therapist.
  • Graduation from an occupational therapy program accredited jointly by AMA and AOTA.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal, organizational, and problem-solving skills.
