📋 Description

\n
• Make home visits to clients in designated geographic territories.
\n
• Evaluate patients’ functional status, including ADL and IADL performance.
\n
• Assess cognitive, psychological, and motor performance; develop individualized intervention plans.
\n
• Evaluate home environment for hazards or barriers to independent living.
\n
• Identify equipment needs and environmental adaptations resulting in enhanced outcomes for clients.
\n
• Continuously assess and revise OT care plans with the nursing team.
\n

🎯 Requirements

\n
• A current Massachusetts Occupational Therapist license.
\n
• Prior home care experience preferred, but not required.
\n
• A minimum of one year of recent work experience as an Occupational Therapist.
\n
• Graduation from an AMA/AOTA-accredited occupational therapy program.
\n
• Ability to work independently and manage time effectively.
\n
• Strong interpersonal skills, organizational skills, and problem solving skills.
\n
• Solid computer skills; EMR experience preferred.
\n
