Occupational Therapist - Home Health

📋 Description

• Make home visits to clients in designated territories.
• Evaluate patients' functional status including muscle function, endurance, vision, and ADL/IADL.
• Assess cognitive, psychological and motor aspects; develop intervention plans.
• Evaluate home environment for hazards or barriers to independence.
• Identify equipment needs and environmental adaptations.
• Continually assess and revise the OT care plan and coordinate with nursing.

🎯 Requirements

• NJ Occupational Therapist license.
• Home care experience preferred, but not required.
• At least one year of OT experience.
• Degree from AMA/AOTA-accredited OT program (transcript/diploma).
• Ability to work independently and manage time.
• EMR experience preferred.
