Occupational Therapist- Home Health

📋 Description

• Make home visits to clients in designated territories.
• Evaluate functional status, ADL and IADL performance.
• Assess cognitive, psychological and motor performance; plan interventions.
• Evaluate home environment for hazards to independence.
• Identify equipment needs and environmental adaptations.
• Update and revise the OT care plan with nursing as needed.

🎯 Requirements

• Current VA Occupational Therapist license.
• Home care experience preferred, not required.
• Minimum of 1 year OT experience.
• Graduation from AMA/AOTA-accredited OT program.
• Ability to work independently and manage time.
• Strong interpersonal, organizational and problem solving skills.
• EMR experience or solid computer skills.
