📋 Description

• Make home visits to clients in designated territories.
• Evaluate functional status: muscle function, endurance, coordination.
• Assess cognitive, psychological, and motor performance.
• Evaluate home environment for hazards or barriers to independence.
• Identify equipment needs and environmental adaptations.
• Update the OT care plan; coordinate with nursing.

🎯 Requirements

• A current DE Occupational Therapist license.
• One year of OT experience; home care preferred but not required.
• Graduation from an OT program accredited jointly by the AMA and AOTA.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
