Occupational Therapist - Home Health

📋 Description

• Make home visits to clients in designated territories.
• Evaluate patients' functional status (ADL/IADL, endurance, vision).
• Assess cognitive/psychological and motor aspects; plan interventions.
• Review home environment for hazards and independence barriers.
• Identify equipment needs and environmental adaptations.
• Educate patients and families on rehab care and safety.

🎯 Requirements

• A current STATE Occupational Therapist license.
• At least one year of recent Occupational Therapist experience.
• Graduation from an AMA/AOTA accredited Occupational Therapy program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience.
