Make home visits to clients in designated geographic territories.
Perform thorough evaluations of patients' functional status, including ADL/IADL.
Assess cognitive, psychological and motor aspects; develop intervention plans.
Evaluate home environment for hazards to promote independence.
Identify equipment needs and environmental adaptations.
Document observations, interventions, and evaluations in the EMR.

🎯 Requirements

Vermont OT license.
Minimum 1 year OT experience.
Graduation from an OT program accredited by AMA/AOTA.
Ability to work independently and manage time.
Strong interpersonal and organizational skills.
Solid computer skills; EMR experience.
