Occupational Therapist - Home Health

📋 Description

• Make home visits to clients in designated territories.
• Evaluate functional status: muscle function, endurance, coordination, ADL/IADL.
• Assess cognitive, psychological, and motor aspects; develop intervention plans.
• Evaluate home environment for hazards to independence.
• Identify equipment needs and environmental adaptations.
• Document observations and care in EMR via touch pad tablet.

🎯 Requirements

• A current Vermont OT license.
• At least 1 year OT experience; home care preferred.
• Graduation from an accredited OT program.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
