Occupational Therapist - Home Health

📋 Description

• Make home visits to clients in designated territories.
• Evaluate patients' functional status and ADL/IADL performance.
• Assess cognitive, psychological and motor aspects; develop plans.
• Evaluate home environment for hazards and barriers to independence.
• Identify equipment needs and environmental adaptations.
• Revise OT care plan; coordinate with nursing.

🎯 Requirements

• A current Vermont Occupational Therapist license.
• Prior home care experience preferred, but not required.
• Minimum of one year of recent OT work experience.
• Graduation from an OT program accredited by AMA and AOTA.
• Ability to work independently and manage time effectively.
• Solid computer skills; EMR experience preferred.
