📋 Description

• Make home visits to clients in designated territories.
• Evaluate functional status: muscle function, endurance, ADL/IADL.
• Assess cognitive, psychological, and motor performance; plan care.
• Evaluate home hazards and barriers to independence.
• Identify equipment needs and environmental adaptations.
• Update care plan and collaborate with the nursing team.
• Educate patients and families on rehab and independence.
• Document care on a touch-pad tablet.

🎯 Requirements

• Current STATE OT license.
• Prior home care exp preferred but not required.
• Minimum of one year OT experience.
• Accredited OT program completion (AMA/AOTA).
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
