Occupational Therapist

📋 Description

• Perform home visits within designated geographic territories.
• Evaluate patients' functional abilities (ADLs, IADLs, mobility, cognition, safety).
• Develop individualized plans to maximize independence and outcomes.
• Assess home environments and recommend adaptive equipment.
• Collaborate with nursing and other disciplines on care plans.
• Educate patients and families on interventions to improve safety.

🎯 Requirements

• Current New Jersey Occupational Therapist license.
• Minimum of one year of recent experience as an Occupational Therapist.
• Graduate of an accredited Occupational Therapy program.
• Prior home health experience preferred but not required.
• Strong clinical, organizational, communication, and problem-solving skills.
• Ability to work independently and manage time effectively.
