Occupational Therapist

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform evaluations of functional status, including muscle function, endurance, coordination, and ADL/IADL.
• Assess cognitive, psychological and motor performance; develop individualized intervention plans.
• Evaluate home environment for hazards or barriers to independence.
• Identify equipment needs and environmental adaptations to improve outcomes.
• Continually assess and revise OT care plans with nursing in the multidisciplinary team.

🎯 Requirements

• A current New Jersey Occupational Therapist license.
• Prior home care experience preferred, but not required.
• A minimum of one year of recent work experience as an Occupational Therapist.
• Graduation from an accredited occupational therapy program (AMA/AOTA).
• Ability to work independently and manage time effectively.
• Solid computer skills; EMR experience preferred.
