Occupational Therapist

📋 Description

• Make home visits to clients in designated territories.
• Evaluate functional status: muscle function, endurance, vision, ADL/IADL.
• Assess cognitive, psychological and motor aspects; develop plans.
• Evaluate home hazards or barriers to independent living.
• Identify equipment needs and environmental adaptations.
• Continuously assess and revise the OT care plan with the MDT.

🎯 Requirements

• A current AZ OT license.
• Prior home care experience preferred, not required.
• At least one year of OT experience.
• Graduated from AMA/AOTA-accredited OT program.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, problem-solving skills.
