- Make home visits to clients in the assigned geographic territories.
- Evaluate functional status: muscle function, endurance, coordination, communication, and ADL/IADL.
- Assess cognitive, psychological and motor performance; develop intervention plans.
- Evaluate home environments for hazards and barriers to independence.
- Identify equipment needs and environmental adaptations.
- Update and revise the OT care plan; coordinate with nursing.
🎯 Requirements
- A current OH Occupational Therapist license.
- Prior home care experience preferred, but not required.
- Minimum one year of recent OT experience.
- Graduation from an OT program accredited by AMA and AOTA.
- Ability to work independently and manage time effectively.
- Solid computer skills; EMR experience preferred.
