Make home visits to clients in designated territories.
Evaluate functional status incl. muscle function, endurance, coordination, ADL/IADL.
Assess cognitive/psych/motor performance and develop intervention plans.
Evaluate home hazards and barriers to independence.
Identify equipment needs and environmental adaptations.
Educate clients and families on rehabilitation for safe, independent living.

🎯 Requirements

A current SC Occupational Therapist license.
Prior home care exp preferred, but not required.
Minimum 1 year OT experience.
Degree from AMA/AOTA-accredited OT program.
Ability to work independently and manage time.
Strong interpersonal, organizational, problem-solving skills.
Solid computer skills; EMR experience preferred.
