Visit clients in their homes to support recovery after injury, illness, surgery.
Make home visits to clients in designated geographic territories; sign-on bonuses for full-time.
Evaluate patients’ functional status, including muscle function, endurance, ADL/IADL.
Assess cognitive, psychological and motor performance; develop plans.
Evaluate home hazards and barriers to independence.
Identify equipment needs and adaptations.

🎯 Requirements

Active and current DE OT license.
Prior home care experience welcomed, but not required.
Ability to travel locally to cases as assigned.
