Evaluate functional status: muscle, endurance, coordination, ADL/IADL.
Develop and adjust individualized OT plans with the care team.
Assess home hazards and adaptations for safer living.
Identify equipment needs and environmental modifications.
Document care and outcomes in EMR on a touch-pad tablet.

🎯 Requirements

Active and current DE OT license.
Minimum of one year of recent OT experience.
Graduation from an OT program accredited by AMA and AOTA.
Ability to travel locally to cases as assigned.
Ability to work independently and manage time effectively.
Solid computer skills; EMR experience preferred.
