- Coordinate and provide restorative OT services with the IDT
- Work with PACE Physician and rehab staff to maximize independence
- Lower patient volumes; build long-term relationships with participants
- Collaborate with therapists, providers, and disciplines
- Deliver and document interventions per care plans
- Provide active and passive exercise and gait/ADL training
🎯 Requirements
- Bachelor’s degree in occupational therapy; master's preferred
- Two years clinical experience with frail/elderly population preferred
- Valid California OT license or license eligibility
- Must obtain CPR and First Aid within 180 days of hire
- Outpatient/acute/inpatient/skilled nursing experience preferred
