Occupational Therapist

📋 Description

• Coordinate restorative and rehabilitative OT services with the IDT.
• Collaborate with PACE physician, rehab staff, and IDT to maximize independence and ADLs.
• Conduct OT assessments in facility and occasional home visits.
• Actively participate in IDT meetings to develop participant care plans.
• Deliver and document interventions per care plans; provide gait and ADL training.
• Work with Therapy Assistants to implement Functional Maintenance Programs (FMP).

🎯 Requirements

• Bachelor’s degree in occupational therapy; master’s preferred
• Two years in a clinical setting with frail/elderly population preferred
• Valid California Occupational Therapy License, or proof of license eligibility
• CPR and First Aid certification within 180 days of hire; certification on hire preferred
