Occupational Therapist

📋 Description

• Conduct OT assessments in facility and in-home.
• Participate in IDT meetings to develop care plans.
• Deliver and document interventions per care plans.
• Provide active and passive exercise, gait and ADL training.
• Work with Therapy Assistants to implement FMP.

🎯 Requirements

• Bachelor’s in OT; master’s preferred.
• 2 years clinical experience with frail/elderly preferred.
• Valid California OT license or license eligibility.
• CPR/First Aid within 180 days; hire-cert preferred.
• Outpatient/acute/inpatient or SNF experience preferred.
