Nurse Practitioner - Virtual Health Assessment

📋 Description

• Conduct preventive screenings and medication reconciliation; diagnose conditions and refer or treat as needed.
• Outreach chronically ill members using historical and suspected health information.
• Engage members unlikely to see their PCP.
• Identify opportunities to increase access to care with licensed providers.
• Cross-train into urgent care and/or virtual primary care services.
• Deliver virtual assessments, diagnoses, and treatments while complying with applicable laws.

🎯 Requirements

• DNP/FNP/ENP from an accredited program; Board cert (AANP/ANCC).
• Compact RN license; ability to obtain licensure in additional states.
• Licensed in at least 2 of: FL, TX, OH, NJ; or ability to add states.
• 3+ years outpatient Family Medicine/Internal Medicine/Primary Care experience.
• Experience with CDI and/or managing HEDIS quality measures.
