Make home hospice recertification visits in NH.
On-call one weekend per month (Fri 5pm - Mon 8:30am) for symptom management.
Monitor client conditions and report changes to Clinical or Client Services Manager.
Follow up with, execute, and document doctors' orders.
Perform client assessments as necessary.

🎯 Requirements

A current NP license in VT and NH.
Active DEA license in VT and NH.
Minimum one year of recent clinical experience.
Prior home care, hospice, or palliative care experience preferred.
Solid computer skills; EMR experience preferred.
Ability to travel to cases as assigned.
