Conduct home hospice recertification visits in assigned territories.
Monitor client conditions and report changes to Clinical/Client Services Manager.
Follow up, execute, and document doctors' orders.
Perform client assessments as needed.

🎯 Requirements

Current NP license in NH and VT.
Minimum 1 year of clinical (medical/surgical) experience.
Prior home care, hospice or palliative care experience preferred.
Solid computer skills; EMR experience preferred.
Ability to travel to cases as assigned.
Strong interpersonal skills.
