Nurse Manager, RN Home Care- Senior Living

📋 Description

• Lead, inspire and motivate others to provide exceptional care.
• Collaborate with Rehab Manager, Director, and DCO to ensure cohesive field staff.
• Review clinical documentation to ensure consistent, high-quality services.
• Assign appropriate clinicians to each client.
• Develop and update the client's plan of care based on continual assessment.
• Coordinate client care, referrals, and act as internal case manager and advocate.

🎯 Requirements

• 2+ years of experience as an RN in Home Health.
• 2+ years in a leadership/supervisory role.
• Excellent organization and communication skills.
• Desire to lead, inspire, and motivate others.
• Strong knowledge of home health policies and procedures.
