Nurse Care Manager, Transitions of Care

📋 Description

• Manage patient panel care using nursing expertise in value-based care.
• Build trusting relationships with patients and families.
• Advocate for resources post-hospitalization and coordinate with care team.
• Assess needs, including social determinants, to guide care plans.
• Ensure timely follow-ups, medication reconciliation, and referrals.
• Coordinate interdisciplinary teams to ensure continuity of care.

🎯 Requirements

• Associate Nursing degree with active RN license (compact preferred).
• 3+ years care management experience.
• 2+ years Transitions of Care experience.
• Chronic/complex care and utilization management experience.
• MS Office skills (Outlook and Teams).
• Remote-work capable with reliable home internet.
