📋 Description

• Manage patient panel and care needs in value-based care.
• Collaborate with physicians, APPs, and IDTs to coordinate care.
• Support programs like Chronic Complex Care, Transitions of Care, CKD management.
• Fully remote role, MT timezone, 8:30am–5pm MT.
• Focus on patient-centered, holistic kidney care.

🎯 Requirements

• Associate degree in nursing
• Current RN license required; compact license preferred
• Care management experience required
• Certified Case Manager preferred
• MS Office with Outlook and Teams
• Remote readiness: reliable home internet and dedicated workspace
