Nurse Care Manager

📋 Description

• Manage overall care for a patient panel within value-based care.
• Build trusting relationships with patients and families for support.
• Coordinate care plans with Evergreen, JV partners, providers, vendors.
• Assess needs, including social determinants, to inform care plans.
• Ensure timely follow-up, med reconciliation, and referrals.
• Support seamless transitions of care with IDTs to reduce fragmentation.

🎯 Requirements

• Associate Nursing degree with active RN license (compact license preferred)
• 3+ years of care management experience
• 2+ years of Transitions of Care experience
• Chronic and complex care management experience
• Utilization management experience
• Intermediate MS Office skills including Outlook and Teams
• Remote work readiness with home internet (min 25 Mbps down/10 Mbps up); Evergreen provides remote equipment
