NP/PA - Virtual Health Assessment (Part-time)

📋 Description

• Perform preventive care screenings, med reconciliation, and care plans.
• Outreach chronically ill members using historical data.
• Engage members unlikely to see their PCP.
• Improve access to care with licensed providers.
• Cross-train into urgent care and virtual primary care.
• Interact with members via virtual assessment, diagnosis, and treatment.
• Ensure compliance with laws and regulations.
• Other duties as assigned.

🎯 Requirements

• PA or DNP, FNP, ENP from an accredited program
• Board Certification (AANP/ANCC/NCCPA)
• Compact RN (NP only)
• Experience with Clinical Documentation Improvement and HEDIS
• PA: Licensed in GA and one of FL/TX/OH
• NP: Licensed in two of FL/TX/OH/NJ
• Ability to obtain licensure in additional states
• 3+ years in outpatient Family/Internal/Primary Care
