New Grad Physical Therapist, Per Diem Weekends, Home Health

📋 Description

• Make home visits to clients in designated geographic territories.
• Assess mobility, ROM, transfers, gait, balance, strength, and safety.
• Develop and implement individualized PT care plans including manual therapy, gait training, and balance interventions.
• Continually assess and revise the PT care plan; participate with nursing on the multidisciplinary plan.
• Educate clients and families on rehabilitative care to promote health and independence.
• Document observations and interventions using a touchpad tablet.

🎯 Requirements

• Active and current PT license.
• New graduates welcome; strong interest; one year of recent PT experience preferred.
• Previous home health care experience welcomed, but not required. We will train you.
• Ability to travel locally to cases as assigned.
• Hands-on training supplemented with virtual learning.
• Strong communication skills to educate clients and families.
