Make home visits to clients in designated geographic territories.
Perform mobility, strength, balance testing; assess safety.
Implement wound care with evidence-based dressings.
Develop individualized care plans with manual therapy, gait and balance training.
Assess and revise care plans with nursing in the MDT.
Educate clients and families in rehabilitative care and safety.

🎯 Requirements

Active Missouri PT license.
1 year of recent PT experience preferred; new grads welcome.
Home health experience welcomed but not required.
Ability to travel locally to cases.
