Conduct comprehensive assessments and develop individualized care plans
Coordinate home care services and other supports
Attend medical appointments as a client advocate and provide ongoing support
Navigate complex healthcare systems to secure appropriate services, resolve barriers and expedite access to care
Act as a liaison for families at a distance
Arrange, monitor, and adjust in-home services as needed

🎯 Requirements

Masters of Social Worker (MSW) required
Experience in geriatrics or elder care preferred
Strong interpersonal and communication skills
Work independently while collaborating with families and providers
Valid driver's license; company car provided for work-related travel
