Making home visits to clients in designated territories.
Assess social and emotional factors related to clients’ health problems.
Provide clinical social evaluations and plan interventions.
Identify community resource referrals to address the client’s and family’s needs.
Advocate for client and family to navigate the community system.
Document observations and evaluations using a touch pad tablet.

🎯 Requirements

Licensed by the DE Board of Social Work Examiners at LMSW or LISW level.
MSW with at least 2 years of community-based experience.
CSWE accreditation evidenced by transcript or degree.
Home care experience a plus.
Strong English communication; ability to work independently.
Solid computer skills; EMR experience preferred.
