📋 Description

• Making home visits to clients in designated territories.
• Assess social and emotional factors related to health problems.
• Provide clinical social evaluations and plan interventions.
• Identify community resource referrals for client and family needs.
• Advocate for client and family through the community system.
• Develop total care plan with the multidisciplinary BAYADA team.

🎯 Requirements

• Minimum 1 year experience in social work in healthcare.
• MSW with at least 2 years of community-based experience.
• CSWE accreditation evidenced by transcript or degree.
• Strong English reading, writing, and communication.
• Ability to work independently and manage time effectively.
• Solid computer skills with EMR experience preferred.
