Medical Social Worker, LSW or LCSW

📋 Description

• Making home visits to clients in designated territories.
• Assist our team in understanding social and emotional factors related to clients' health problems.
• Provide clinical social evaluations and plan interventions based on evaluation findings.
• Identify community resource referrals to address client and family practical and environmental needs.
• Act as an advocate for both client and family to navigate the community system.
• Develop and maintain contracts with public and private agencies as resources for the patient.

🎯 Requirements

• Licensed by NJ Board of Social Work Examiners at LMSW or LISW.
• Master's in Social Work with at least 2 years' community-based experience.
• CSWE accreditation evidenced by transcript or degree.
• Ability to work independently and manage time effectively.
• Strong computer skills; EMR experience preferred.
• Ability to travel to cases as assigned.
