📋 Description

• Making home visits to clients in designated territories.
• Help team understand social and emotional factors of client health problems.
• Provide social evaluations and plan interventions based on findings.
• Identify community resources to meet client and family needs.
• Advocate for client and family to navigate the community system.
• Contribute to the total care plan with the multidisciplinary BAYADA team.

🎯 Requirements

• Licensed by the PA Board of Social Work Examiners at the Master (LMSW) or Independent (LISW) level.
• Masters in Social Work with at least 2 years of community-based experience.
• Accreditation by the Council on Social Work Education, shown by transcript/degree.
• Demonstrated ability to read, write, and communicate in English.
• Work independently and manage time effectively.
• Solid computer skills; EMR experience preferred.
