📋 Description

• Making home visits to clients in designated geographic territories.
• Assist team in understanding social and emotional factors related to clients' health problems.
• Provide clinical social evaluations and plan interventions based on evaluation findings.
• Identify community resource referrals to address client and family needs.
• Act as an advocate for client and family to navigate the community system.
• Participate in the total plan of care with the BAYADA clinical team.

🎯 Requirements

• Licensed by the NJ Board of Social Work Examiners at LMSW or LISW level.
• Master of Social Work with at least 2 years of community-based experience.
• CSWE accreditation evidenced by transcript or degree.
• Home care experience a plus.
• Ability to read, write, and communicate in English.
• Ability to work independently and manage time effectively.
