📋 Description

• Make home visits to clients in designated geographic territories.
• Assess social and emotional factors related to clients' health problems.
• Provide social work evaluations and plan interventions.
• Identify community resource referrals to address patient and family needs.
• Advocate for clients to navigate the community system.
• Document observations and care activities using an electronic device.

🎯 Requirements

• Master's in Social Work with at least 2 years of community-based experience.
• CSWE accreditation evidenced by transcript or degree.
• Home care experience a plus.
• Proficient English communication (read/write).
• Solid computer skills; EMR experience preferred.
• Ability to travel to cases as assigned.
