📋 Description

• Making home visits to clients in designated geographic territories.
• Help the team understand social and emotional factors affecting clients’ health.
• Provide clinical social evaluations and plan interventions based on findings.
• Identify community resource referrals to meet clients’ and families’ needs.
• Advocate for clients and families to navigate community services.
• Contribute to the total care plan with the multidisciplinary BAYADA team.

🎯 Requirements

• Licensed by NJ Board of Social Work Examiners (LMSW or LISW).
• MSW with at least 2 years in community-based work.
• CSWE accreditation demonstrated by transcript/degree.
• Home care experience preferred.
• Strong English reading/writing and communication.
• Ability to work independently and manage time.
