Medical Social Worker, Hospice

📋 Description

• Making home visits to clients in designated territories.
• Assess social and emotional factors related to health problems.
• Provide social evaluations and plan interventions.
• Identify community resources for client and family needs.
• Advocate for clients and families through the community system.
• Document observations and interventions on a touch pad tablet.

🎯 Requirements

• Licensed LMSW or LISW by PA Board of Social Work Examiners.
• MSW with at least 2 years of community-based experience.
• CSWE accreditation evidenced by transcript or degree.
• Home care experience a plus.
• Excellent English communication; strong interpersonal skills.
• Ability to work independently and travel as needed; EMR experience preferred.
